List of required documents to apply for a U.S. nonimmigrant visa

The following documents are required for business and tourist class of nonimmigrant visas:

1.  Electronic application form DS-156. This application must be filled out electronically at http://evisaforms.state.gov. This application must be printed and signed by the applicant.  It will also be necessary to make an appointment for an interview. Please make sure that the barcode of the application is printed clearly. See the example below:

2.  All male applicants between the ages of 16 and 45 must complete the application form DS-157. This application may be printed out from this webpage and filled out manually.

3.  A passport valid for travel abroad.

4.  A photograph taken within the past 6 months (photographs must be in color, 2”x2”in size and on a white background). Only original photographs are acceptable. Photos that have been altered in any way (copied, digitally scanned, retouched, etc.) will not be accepted. For more information, please click here.

Student and exchange visitor visa applicants and their dependent family members (F-1, M-1, J-1 and their dependent family members)

All documents required for tourist and business class visa applications and the following documents:

1.  Form DS-158. This form may be printed out from this webpage and completed manually.

2.  Form I-20 for student visas (F-1, M-1, F-2 and M-2) and DS-2019 for exchange visitor visas (J-1 and J-2). These forms are issued by the educational institution and the sponsoring organization respectively.

3.  Form I-901 or I -797 to prove the SEVIS payment. See this link on SEVIS payment in general and to find out what categories of applicants are exempt from this payment. The only way to pay this fee in Uzbekistan is through a credit card online at http://www.fmjfee.com/. Applicants who do not present proof of SEVIS fee payment will not be interviewed.

4.  A Training/Internship Placement Plan, Form DS-7002. All exchange visitor (J visa) trainee or intern visa applicants with DS-2019 forms dated on or after July 19, 2007 (based on Box 7 on form) must also present Training/Internship Placement Plan, Form DS-7002 when applying for their visa. If your Form DS-2019 is issued prior to July 19, 2007 a Form DS-7002 is not required. Temporary worker visa applicants (H, L, P and O visas)

All documents required for tourist and business class visa applications and the following documents:

1.  Petition – Form I-129 or the notice of action – Form I-797. This document filed in the Department of Homeland Security by the prospective employer. 

2.  If the above forms are not available, the applicants must contact the Consular Section via E-mail and provide the petition number. The Consular Section will inform the applicant if the petition information has been made available to the Consular Section through internal channels. If the petition information is available in Consular Systems, the applicants are allowed to apply without the I-129 or I-797.
