Lost and Stolen Green Cards

The Consular Section of the U.S. Embassy in Uzbekistan processes applications from Lawful Permanent Residents (LPRs) with lost or stolen I-551 forms (Permanent Resident Card) by appointment only. 

After DHS verifies the status of the alien seeking a reentry permit, the Consular Section will issue a transportation letter.

Required documents

  1. Passport or other photo identification indicating the alien’s name: original and photocopy;
  2. Evidence that the alien did not abandon his/her residence in the United States for more than one year. Plane tickets and passport entry stamps may serve as supporting evidence;
  3. Two recent photos: 2”’x2” size; background must be white or off-white;
  4. $165 fee for the transportation letter.
